Appellant has filed a motion for waiver of costs associated with the preparation and delivery of transcripts, as well as a transcript request form. The motion requests production of transcripts of proceedings conducted on March 12, 2024, and April 16, 2025. However, the district court docket and minute entries do not indicate that any proceedings occurred on those dates. Accordingly, the motion is denied. Appellant's transcript request form is deficient because it is not addressed to the responsible court recorder. The clerk shall strike the request form filed on October 15, 2025. On September 25, 2025, this court entered an order directing appellant, among other things, to file the docketing statement by October 16, 2025. See NRAP 14(b).
